Please choose from the list with descriptions below or from the drop down menu here:

  • Legal Note* - Must be filled out with each new Program Application.

  • Program Application* - Must be filled out to begin a new program with CDP.

  • Transaction Forms:

  • Independent Contractor* - Must be used along with a Form W-9 for hourly work performed for a program.

    • Form W-9 - Gathers pertinent information to hire an individual as a contractor to a program.

  • Payroll Employee Form* - Must be used along with Form I-9 and Form W-4 to be placed on bi-weekly payroll for a program
    .
    • Form I-9 - Gathers information to hire an individual to be placed on bi-weekly payroll for a program.

    • Form W-4 - Gathers information to hire an individual to be placed on bi-weekly payroll for a program.

  • Credit Card Donation Form - To be used for manual credit card donations to a program. Must be signed by the donor and the Program Manager.

  • Travel Request Forms:

    • Before the Trip* - Must be filled out prior to taking a trip for program related activity.

    • After the Trip* - Must be filled out after a trip and in conjunction with the Before the Trip Form for prior approval to obtain travel reimbursement.

    • The current mileage reimbursement rate is 50.5 cents per mile.

  • Sample Donation Receipt Letter* - Use this form to confirm a donation to your program if the amount is under $250. If the amount is $250 or above, CDP will automatically generate and mail a receipt to the donor. This form can be used to prove a donation for tax deduction purposes.

  • Grant Approval Request Form* - Use this form to make grant recommendations to individuals who require charitable financial assistance but are not able to work for taxable income. Please Click Here to view the current Federal Poverty Guidelines

  • Petty Cash Approval Form* - Petty Cash funds may be used only for "small purchases" for the program, not to exceed a "reasonable" limit per purchase. Small purchases include postage, stamps, envelopes, etc.

  • Petty Cash Reconciliation Form* - To reconcile expenditures which have been made from the petty cash fund, the Program Manager must fill out this form and accompany it with original receipts for which the funds were used.

  • Fundraising Event Approval Form* - Submit this form if holding an event for your program. CDP will not approve any raffles or bingo games (or any other type of event which may be construed as gambling) as fundraising events.

    • IRS Form 8283 - Used in conjunction with Fundraiser Approval Form when accepting non-cash gifts.

    • IRS Form 8282 - Used in conjunction with Fundraiser  Approval Form when donated property is sold or otherwise disposed of.

  • Non-Cash Gift Form* - Complete and submit this form to CDP for any Non-Cash Gift transations. Click here to view our current Non-Cash Gift Policy. When necessary use IRS Forms 8283 and 8282 (above) in conjunction with this Form.
    Additionally, please review this recently published IRS Publication "Determining the Value of Donated Property."

  • Scholarship Approval Form* - Must be used when setting up scholarship programs. Please review our strict scholarship guidelines by clicking here.

  • Successor Instructions Form* - Use this form to name an individual as the successor to your Program in case of disability or death or to have the Program terminated and remaining funds be donated to a selected charity.

  • NACEC Application - To help others achieve the success and satisfaction you receive by operating a Field of Interest Fund please consider becoming an active member of our
    National Association of Charitable Estate Counselors.
